Challenge to CKD - From Japan to World

Japan CKD Initiative has started in 2004. The executive committee of Japanese Society of Nephrology (JSN) has decided to establish a committee for CKD Initiative (JSN-CKDI). The followings have been the 4 main activities of JSN-CKDI; (1) Evaluation and estimation of prevalence and co-morbidities of CKD patients in Japan, (2) Creation of clinical guidelines for primary care physicians to treat Japanese CKD patients, (3) Organization of a common platform on which number of different communities can appeal the importance of CKD to the public, (4) Exchange of the information and experience of CKDI in different countries to form an alliance against CKD. JSN-CKDI has estimated the prevalence of CKD stage 3-5 (GFR less than 60 ml/min/1.73m2) as 18.7% of general population, but the risk of GFR reduction rate (more than 2 folds) was seen in the population with GFR less than 50. It was also put in question whether MDRD equation for estimated GFR suited for Japanese or Asian. A clinical guideline for CKD is now under way. One of the most exciting events since 2004 was the establishment of The Japan Association for CKD Initiative (JCKDI). The core members of JCKDI are JSN, The Japanese Society of Dialysis Therapy (JSDT), and The Japanese Society of Pediatric Nephrology (JSPN). The Kidney Foundation Japan (KFJ) and International Kidney Evaluation Association Japan (IKEAJ) have joined as observers. The Japan Ministry of Health, Welfare and Labor (MHWL) has recently been interested in starting a strategic outcome study to manage CKD. From these backgrounds, many of JSN members are now realizing the importance of collaborative action against CKD in Asia. The main event following this line is the Asian Forum on CKD Initiative held in Hamamatsu, Japan.
