Background
Cancer is associated with patient weakness and severe illness because
the metastatic diseasesis and loss of appetite. The nephrological complications
include renal failure (ARF, Acute on chronic ARF or CKD). In the past
hemodialysis was a relative contra indication for cancer sufferers especially
those with metastic disease. Nowadays hemodialysis is used for other indications:
drug dosing and improving the quality of life before death.
The aim of this study is to evaluate the use of conventional hemodialysis
(cHD) in terminal stages of cancer.
Methodse and Results
The cHD treatments were performed adjusted to individual patient requirements.
There were 55 patients with cancer who were dialyzed; the most frequent
diagnosis was obstructive nephropathy due to cervical cancer (> stage
III (post obstructive)). Only 8 patients suffered nephrotoxic injury due
to cisplatin or radiation. In these patients cHD was performed if the
treatments with hydration and N acethyl cystein were incomplete and were
aimed at increasing the renal function. The most common complication in
the cancer patients were hypoalbuminemia (pre cHD) due to poor food intake
or hypotension due to heart disease (pre or during cHD). Laboratory parameters
: blood urea and serum creatinine levels were successfully decreased by
the treatments but most of them increase again rapidly.
The most cHD treatments were performed 3 times with a maximum ofal 10
times before dieddeath..
Conclusion
cHD in terminal malignancy patients was observed to not prolong life and
cHD clearly has limitations due to the poor general condition of the patients.
